Male erectile dysfunction (ED) increases with aging and affects millions. Vascular and/or peripheral and central neurogenic factors are the most frequent causes of ED. In the last two decades, different physiological mechanisms regulating penile erection at local level and, to a lesser extent, at central level, have been identified and characterized. This has led not only to the discovery of peripherally and centrally active drugs, but also to alternative therapeutic strategies for ED.
