Pulmonary hydatid cysts whose diameter is greater than 10 centimetres are defined "gigantic" and pose peculiar problems regarding both surgical treatment and postoperative care. In fact the postoperative period often is longer and complications are more frequent than usual. The Authors present a case regarding a 75 years old man with a gigantic hydatid cyst of the left lung, determining compression and displacement of mediastinum and trachea. The patient underwent surgical treatment consisting of removal of the cyst and lobectomy and recovered well. He was discharged in 38th postoperative day.
