A 61-year-old man presented with a relapse of idiopathic pulmonary hemosiderosis recalcitrant to cortisone therapy which had resolved the initial episode of 26 years earlier. Immunosuppressant treatment led to rapid and complete control of this second episode. The clinical course of this case clearly supports our argument for an autoimmune etiopathogenesis of this disease
